CAV of Academic Records

Academic records, if to be used for employment purposes abroad, need to go through the CAV process; that's Certification, Authentication and Verification.

DepED process: this is when you are authenticating high school records.

STANDARDIZED PROCESS FLOW
STEP 1 
Secure documents where you graduated / attended. 
A. For Public School Graduates: 

  • Student permanent records (F-137)
  • Diploma
  • Certification of Graduation issued by school head.  
B. For Private School Graduates: 
  • Student permanent records (F-137)
  • Diploma
  • Certification of Graduation issued by school head
  • Special Order for Secondary Graduates issued by the Division Office for two-level schools or by the Regional Office for three-level schools.  
C. For Graduates of Private Schools with Accreditation from Philippine Accrediting Association of Schools, Colleges and Universities (PAASCU)
  • Student permanent records (F-137)
  • Diploma
  • Certification of Graduation issued by school head
  • Certification of Accreditation from PAASCU

Applicants who fail to complete their primary or secondary level curriculum level (Private / Public) should secure the following documents from the school they enrolled: 
  • Certification of grade / year level completed
  • Student permanent records (F-137)
If the documents are to be used for enrollment abroad, the applicant shall secure the following from the school they enrolled: 
  • Student permanent records (F-137)
  • Certification of grade / year level completed

STEP 2
The applicant should submit a filled application form with complete requirements from the school attended to the Regional Office, Records Unit.  You can download a form from HERE


After the receiving clerk sees the completeness of the requirements you will be issued a claim stub and advised to claim red ribboned academic records from DFA.  It should only take 5 business days from the date of transmittal.  


The regional office will verify authenticity of your documents and if no discrepancy are found then you should be able to claim you records, already red ribboned from the DFA, in 5 business days as advised.  


REMEMBER: 
Application may be filled by the applicant or a duly sworn representative.  In which case you need to furnish your representative with a Power of Attorney.  Be sure to have complete requirements before submitting your application.  


FOR COLLEGE ACADEMIC RECORDS:  
You only need to go to the school / university where you attended/ enrolled and submit an application form.  The school itself should take care of the transmittal to the corresponding verification office.  You will be advised by your school when you can claim your red ribboned records.  Do not forget your claim stub.
